The Homebound Program is provided only in unusual circumstances in which a student's absences are recurrent or extended because of illness or accident.
Homebound instruction will be made available in Language Arts, Social Studies, Mathematics, and Science. Request for homebound instruction in other areas will be given consideration, depending upon the circumstances and the availability of certified staff members to teach in the subject area.
1. Upon request homebound educational services are provided for students who have missed or who will be unable to attend classes for five (5) consecutive school days.
2. The school district requires an approved, signed statement from the treating physician verifying the illness and the term of recuperation.
3. A recommendation is required from the Principal of the school in which the student is attending. The Application for Homebound Instruction Form must be forwarded to the Assistant Superintendent, who will determine if homebound instruction is appropriate.
4. Upon approval from the Assistant Superintendent, the homebound teacher will set up a schedule of instructional time to the satisfaction of all parties involved.
5. A follow-up statement provided by the treating physician verifying the need for continued services shall be required every 30 days. It will be the responsibility of a school representative to contact parents to secure this documentation.
